3. Authorized agent certificate
In the event that a foreign enterprise has no office or branch office in China or if there is a special reason (it shall provide a description of the situation), it shall entrust the enterprise with the after-sales service capability to submit the application as the applicant unit when entrusting other enterprises to handle the application. At this time, prove.
4. Quality management system certification (ISO9001-2000 standard) certificate copy (already obtained)
If the applicant unit is not the same, if the applicant unit is the agent or the distribution unit, it shall submit the certification certificate of the production unit. If the production mode of the applicant unit is OEM, two units of the system certification certificate shall be provided. The scope of the quality management system certificate should cover the product of the product application.
5. Apply for the organization's quality management manual
When the applicant unit and the production unit are inconsistent, if the applicant unit is the agent or the distribution unit, the quality management manual of the production unit shall be submitted. If the production mode of the applicant unit is OEM, two units of quality management manual shall be provided. If the TLC quality management system certification, may not provide.
6. Application for product certification of the product introduction, the production process, the implementation of standards, product description report, the main production equipment and testing equipment list
Product description report, the main production equipment and testing equipment list in our center site has a standard form; if the implementation of industry standards, no longer provide additional standards.
7. Inspection reports on key components and materials
The type of test and report on the material of any key components and materials provided in the product description or the organization's technical report on the material or the organization's type test report with its material made into the finished product (this type of test report shall indicate The use of key components and materials for the supply side)
8. Company profile
Applicants, the production unit is inconsistent when the need to submit two units of the enterprise profile and product after-sales service commitment (if the after-sales service by the applicant unit is responsible for the application unit to provide after-sales service commitment; if the service unit is responsible for the production unit, The agreement between the unit and the production unit for after - sales service).
9. Certificate of registration
If you have a formal registration of the trademark, please provide a copy of the corresponding trademark registration certificate. If the applicant unit and the trademark owner are inconsistent, should provide the corresponding trademark licensing agreement.

3, all the application materials should be carefully prepared, shall not be altered, should ensure that true and effective.
4, the application materials which need to be stamped with official seals, should be provided with the official seal after the original.
5, the application materials need to sign the legal person, the business license should be marked by the legal representative of the signature, or by the legal representative of the authorized person to sign, but should be issued by the original legal representative or a copy of the power of attorney The official seal of the applicant unit can be signed by authorized personnel.
6, when the applicant's communication address, contact person, telephone and fax changes should be notified in writing to the center.

A, application form

3.4 "According to the standard": should fill out the production of product certification products used by the technical standard name and standard number, if the use of multiple standards, are listed one by one.
3.5 "Registered Address": should be filled in by the address in the legal person's business license.
3.6 "Trademark": should fill in the country has registered the trademark, has not yet been formally registered or no trademark should be filled in this column "no".
3.7 "Product name and model / specification": Product name and model / specification shall comply with the requirements of "Product name, unit division" in Annex 1 of the Certification Implementation Rules. Different certification units should fill out the product certification application form.
3.8 "Select the detection subcontracting mechanism": Please select and fill in the name of the organization that undertakes the product of the certification product according to the detection range of each inspection subcontractor in the "TLC product certification business scope and the contract testing organization list".
B, corporate legal person business license or registration certificate copy

C, authorized agent to prove
1, the overseas production units in the application for product certification, should be in China in the office, branch offices as an application to submit an application, this time does not require "authorized agent certificate." D, quality management system certification (GB / T19001-2008) certificate
1, the enterprise should provide a certificate issued by the issuing authority to confirm the seal or seal of the applicant GB / T19001-2008 certificate
2, the certification certificate in the scope of certification should be applied for product certification products.
3, the certificate should be provided within the validity period.
E, apply for the organization's quality manual
Applicants should provide the organization's quality manual. If the applicant unit is not the same, if the applicant unit is the agent or the distribution unit, the quality management system file of the production unit shall be submitted. If the production mode of the applicant unit is OEM, two units of quality management system documents shall be provided. Quality manuals are not available through the TLC Certification Center (TLC) quality management system.
F, product introduction, production process, implementation standard, product description report, main production equipment and testing equipment list
1, the product description should be applied to apply for product certification product name, model and fill in the application form with the name, model. Product introduction need to be stamped with the official seal of the applicant.

2, product description report
"Key components and material information" should be consistent with the contents of the form, can not be changed. If its products do not contain a certain key components and materials, should be marked "no". All types of key components and materials should be filled in complete, the supplier should include all the organization's suppliers, can not be missing. When the supplier is an agent or a dealer, it shall fill in the production unit of the key components and materials, not the agent or the dealer. The name of the English abbreviation should specify the Chinese name and the English name.
3, production equipment: should be introduced in the production of the product certification products, the use of the production equipment, specify the production equipment used in the name, model, quantity, origin and all other information, should fill in a complete list of production equipment Can be downloaded from the TLC website "Product Certification" package;
4, testing equipment: should be introduced in the application of product certification products, the use of instrumentation, specify the instrument used in the name, model, quantity, origin and all other information, should fill in a complete list of testing equipment Can be downloaded from the TLC website "Product Certification" package;
The above materials need to be stamped with the official seal of the applicant.
G, other information required by the product certification application
Such as: product photos, corporate standards. Product photo that is the product appearance, the internal structure of the photo should be more than five inches color or color printer to print the picture, the photo requirements from multi-faceted, multi-level shooting, such as panels, various modules and various interfaces, etc., to fully reflect the application In the case of a certified product, the model shall reflect the model of the product certification equipment and conform to the application form.
H, business situation
Enterprise situation, including the basic profile of the enterprise, production conditions, instrumentation, quality management system, after-sales service and other specific measures related content. Applicants, manufacturers do not agree to submit two units of enterprises to introduce and provide the two sides signed the latest processing agreement. If the applicant unit, manufacturing enterprises for the relationship between the head office and the subsidiary, the need to provide a two-state relationship between the latest situation.
1, the basic profile of the enterprise: should introduce the establishment of enterprises, the number of enterprises, plant area, registered capital, fixed assets, production capacity and the main products and other information;
2, the quality management system: should detail the quality assurance system to establish the time, running time, operation, the company's current organizational chart, when through which certification and certification through the annual supervision and other content;
3, after-sales service: should be detailed in the sale of the product certification products, the basic situation of after-sales service, including after-sales service commitment or effective after-sales service flow chart. Belong to the provisions of the State Three Guarantees (Bao Xiu, Baohuan, Baotui) products, should provide the implementation of three bags of commitment and marked three packs of time and conditions. The commitment of the enterprise shall be consistent with or exceed the state regulations.

I, the key components and materials inspection report
In accordance with the "product description" filled in the "key components and materials information", should provide all the key components and materials for all types of test report (which can be provided by the supplier, or can organize their own commission test) ; or the organization of their own key components and materials for the process of testing the process test report; or made of the finished product after the finished product type test report (finished type test report should indicate the use of key components and materials supplier).
